Harbor-appointed pilots were taking the ship into open water. They're partially at fault, and the rest of the blame lays on the shipping company for maintenance issues, should they exist.

The Indian crew, as much as a joke as that is, did the needful and called mayday in time to stop traffic on the bridge.
