They always try to mask the death - suicide, accident or illness. That's why the CIA researched the simian virus that caused cancer in polio vaccinated: giving somebody cancer by infecting them with a virus allows for a lot of plausible deniability. To mask a death as C19 death, the symptoms must be there (oxygen level below 80 and so), and the tests must be positive - this is most likely done by the real virus, everything else would be very complicated. They will use one of the more deadly variants, and it plays into their hands that patients are not allowed to know which variant they got.
