They get it because of lipid escape, just because your nurse or tech aspirates the needle doesn't mean your immune system wont take debris from the wound and let it into your blood stream.

Like the blood clotting, myocarditis is most likely caused by antibodies. Clotting and myocarditis come with the second shot, when the immune system has already learned to create the antibodies in no time and huge amounts.
