I'd put money on it that the customer had a diversity hire install the heads and didn't bother to make sure the lifter was working properly on that intake valve. Sure does look like the valve was left open at least a little bit by the looks of the carbon deposits around the intake. That company is going to pay a high price for their diversity hire and they are no longer a customer of the guy who clearly knows what he's doing. Never let DEI pets work on your engines.
