Although often blamed on a single employee stealing an (incomplete) Rubycon electrolyte formula, the video questions this narrative, as the problem was too widespread.
Once a Chinese company gets something like this, it seems to get shared everywhere with every little company getting the same formula - how many companies stole it from the thief?
I can totally believe it was one source that spread everywhere.

That’s correct. IIRC the CCP banned use of some brands because of fire hazards their products could cause.
The tantalum capacitors also had their firecracker period.
