Japan is sort of known for having bad teeth, that said its mostly from dental overcrowding because their diet consists of soft foods.

No, fluoride does work topically, just not if you ingest it, even countries which use fluoride alternatives admit fluoride works to harden dentin and repair minor wear.
I've been on RO water for 20yrs and fluoride free toothpaste for 10, no cavities. I'm guessing you're ok having fluoride toothpaste if you don't digest it, but I'd never drink it. Worried about your teeth? Drink milk.

Do you add any minerals back to your RO water? I was under the impression that ultra pure water (close to 0.00 TDS) is reactive and will leech ions and shit.
Nah, I take vitamins and iron supplements. I just stay away from distilled water
