I have stage 4 emphysema, prognosis is very poor. The doctor started me on these meds, but they cost 3,000/month without insurance, and I don't want to be dependent on these meds anyways. I have found exercise works really well, as does a modified meat only/keto diet with fruits. What else can I do?

As a last ditch effort you can try the 40 day water only diet. An extreme measure.

This forces your body to burn body fat for fuel, and that fat is converted into acetone instead of glucose. (Think rocket fuel.) This not only starves cancer cells, it triggers a general cell die off in your body. It looks bad the first twenty days. However, by around day 30, the body produces all new cells, which reduces your physical age by about ten years. Hopefully the cancer will have died off in the process. You will feel fantastic.

At the end of that your body weight will be down 20 to 25% with a very low fat content, and all shiny new skin. Return to eating by sticking to a lower carb diet, with only natural sugars, like those found in milk and fruit. Complex carbohydrates

Cancer will ferment carbon in the absence of other sources of energy. So no, this will not work for tumors.
