I was sent to Japan to do some IT work. They made sure that a foreigner was on every shift for the install of new equipment. Why? Cause the customer would demand the firing of employees who would make mistakes. The blame would be placed on the foreigner and fired (never to return to Japan for work). This idea of holding people accountable for the work they do is still strong in Japan.

This doesn't seem like it's holding anyone accountable and merely just having a scapegoat to lay the blame on. If they held the responsible party accountable then you could say this. Asians love the idea of saving face which has lead to many an innocent person to suffer for the actions of others....
