We've tried nothing and we're out of ideas ;) We haven't been to the vet, due to circumstances beyond my control, don't ask.

Just wanted to jump in amd echo that it seems to be an insect or parasite thing. Dawn dish soap, the blue shit, works amazingly to kill fleas etc. That, some of the cream mentioned above and the temporary use of the cone of shame may get you where you'd like to be. The cones are usually inexpensive and can be found at any pet supply dealer, online or otherwise. I hope you get success with your little friend there bud.

^ Can confirm this works. Did it with one of my own cats that had a skin rash (vet didn't know cause and just gave her steroids, which worked for a while but it would always come back). After I gave her a bath with the dish soap it has rarely come back at all, and is very mild when it does.
