Word on the Internet is it was pilot error. Either overweight or misconfigured for take off. I tend to agree that it looked like the flaps were not extended for takeoff.

Yeah it looked like he rotated kind of late too, all the dust at the end of the runway. It certainly didn't gain any altitude.
