There are, because rails can be poorly maintained, engineers and conductors are pressed to do things quicker, and shippers generally don't give a shit about what or how they ship things.

The Ohio one, however, was a comedy of errors. Highly dangerous chemicals being shipped without hazmat notices, at too high of a speed, on fire says there were failings on all parts. This is criminal negligence at best.
