assume that basic firearm handling skills are part of Hollywierd's on-set protocols.

You would be wrong. The 'Armorer' is responsible for maintaining safety. Actors/Actresses get to act like children. If a standard NRA firearm safety course was mandatory for everyone there this would never have happened.
