Nicotine it self is a carcinogen and the scientific community and even the tobacco industry do not dispute this. Why do you think you have to move patches around or else you get a rash... nicotine is inherently toxic. It is a poison the plant produces to deter animals from eating it. Of course if you do a quick search on this you will see lots of fake news claiming it is safe... but then you can go and look at public court docs where everyone agrees on the science. People are allowed to lie on the internet... lying in court however is a different matter. The issue is that smoke has other more toxic carcinogen as well so things like the patch and vapes are considered safer and a valid mechanism to get people to quite smoking so they are allowed on the market.
