I've seen a video of a women using a shorter ladder inside. She fell face down, striking a counter with her head as she fell. She broke her neck and was dead before she hit the ground.
Simple rules. Keep center of gravity over ladder. Never reach. If on top three steps someone must hold ladder. If extension ladder, it must be held or anchored. Both preferred.
Women domain is inside. Men's outside. Help outside should always be appreciated and supervised.

Yep, I'm going to agree with you, and I disagree that some are saying it's a joke too, it's not, it's saying get back in the kitchen and stop helping.
Men fall off ladders all the time, I have too, and 9/10 it's because we have some bravado about getting the job done. In fact the last time I fell off, my wife was actually in the kitchen making me a sandwich. If she had wanted to help instead then I'd probably have been less sloppy about securing the ladder.
