What's going on?

  1. Ohio is still fairly industrialized, and there's always been a lot of chemical manufacturing here.
  2. Companies are cutting maintenance to the bone, and beyond.
  3. ???

With falling maintenance levels, yes - and it's probably going to be happening more and more.

Interesting. You'd think it would be less expensive to do regular maintenance to prevent these "accidents" that need to be triaged later. However, I recognize the short term gain of delaying maintenance until after the fiscal year to make the CEO's bonus look better.
