Those are: "health plans, health care clearinghouses, and health care providers that conduct standard electronic transactions" Your employer is not one of the covered entities. Neither is a business that you wish to visit. If your employer calls your doctor to ask if you're vaccinated, that's prohibited because you doctor is a Covered Entity under the privacy rule.
